Health and wellbeing see patients who have non-medical problems and issues that need additional help. They operate from the CAB (citizen advice bureau) in Kidderminster on New Road, but come in to see patients at York house on a Friday morning.

They also offer help to patients who would like to make a lifestyle change and interested in exercise classed and support groups, and also if they are interested in volunteering.

Self-referrals are available for the Health and wellbeing service. The forms are behind reception to be collected and filled out. Alternatively, please click here to download a form to send to the surgery. Once the relevant information has been filled in, your referral will be passed to Chloe, Practice Administrator, who will contact you to book an appointment.
